$240 a year or monthly payment plan $20.00
(includes husband, wife, children and non-employed dependents, and cohabiting parents)
$180 a year or monthly payment plan $15.00
(includes their children and non-employed dependents, and cohabiting parents)
$120 a year or monthly payment plan $10.00
(no children, no cohabiting parents, no non-employed dependents)
